Overview

After this course, the nurse will be able to recognize clinical, social, economic, environmental, and structural factors that increase a person’s risk of poor health outcomes. The nurse will use respectful screening, communication assistance, and safety assessment without stereotyping or stigmatizing patients. The nurse will also be able to prioritize urgent needs, collaborate with interdisciplinary and community resources, and evaluate an advocacy plan that protects autonomy, informed decision-making, continuity of care, and equitable access.

Objectives

Upon completion of this independent study, the learner will be able to:

  1. Differentiate individual risk factors, population-level disparities, and structural barriers that contribute to vulnerability.
  2. Assess patients for urgent clinical needs and common barriers involving safety, housing, food, transportation, communication, medication access, and continuity of care.
  3. Apply evidence-informed nursing approaches when caring for selected populations experiencing elevated health risks.
  4. Demonstrate culturally and linguistically responsive communication, including appropriate use of qualified interpreters and health literacy precautions.
  5. Develop and evaluate a patient-directed advocacy plan that supports autonomy, safety, access, and continuity of care.
  6. Document vulnerability-related findings and referrals using objective, person-first, nonstigmatizing language.
