- Quantify fall risk for older adults using current national data and explain why most falls go unreported.
- Build a fall-reduction plan combining medication review, strength and balance exercise, vision checks and home modification.
- Reconcile medicines in a home where the list, the bottles and the patient's practice may all disagree.
- Apply hand hygiene correctly in a home without clinical sinks, including when alcohol-based sanitizer is appropriate.
- Identify catheter-related infection risk and recognise features suggesting infection beyond the bladder.
- Deliver pressure injury prevention using repositioning, skin care and pressure-relieving products.
- Teach home oxygen safety, including the fire risk and the prescription-only nature of oxygen therapy.
- Support and assess the family caregiver as a member of the care team rather than an unpaid extension of it.
